ShareBackground:Sildenafil, the active ingredient in Viagra, is a phosphodiesterase type 5 (PDE5) inhibitor. It is widely used to treat erectile dysfunction (ED) in men and pulmonary arterial hypertension (PAH). However, its efficacy and safety have not been established. This study aimed to assess the safety and efficacy of sildenafil (Viagra) in the treatment of ED in men with PAH, and to determine its molecular structure and pharmacological effect.
Methods:Sildenafil and its molecular structure were investigated usingin vitroandin vivostudies. PDE5 and PDE6 were blocked with 10%sildenafilin vitro and PDE5 and PDE6 inhibitor sildenafil was used in the study. The results showed that sildenafil inhibited PDE5 activity, which was enhanced by PDE5 and PDE6 inhibition, as well as sildenafil (10%). It is considered that PDE5 can increase cyclic GMP secretion from smooth muscle and increase cyclic GMP concentrations in the peripheral vascular endothelium. Sildenafil inhibited the PDE6 activity of the smooth muscle, which was enhanced by PDE6 and PDE5 inhibition. The inhibitory effect of sildenafil was more potent than that of PDE5 and PDE6 inhibition. This study was approved by the Institutional Review Board of Xiangya Hospital, Capital Medical University (IRB No. H-15-0206).
Results:Sildenafil inhibited the PDE5 activity of the smooth muscle of the vascular smooth muscle, increased PDE6 activity, and inhibited the PDE5 activity of the smooth muscle of peripheral vascular endothelium.
